[SVGMobile12] Appendices J and K lack hyperlinks to element/property definitions

Appendices J (Elements Table) and K (Attributes and Properties Tables)
in http://www.w3.org/TR/2005/WD-SVGMobile12-20050413/ lack hyperlinks to
the definitions of the elements or properties listed.  Such hyperlinks
made the equivalent tables in Appendices L and N of
http://www.w3.org/TR/2003/REC-SVG11-20030114/ extremely useful.  I can't
find a replacement that lists all the elements or properties with links
to their definitions.  Links to attribute definitions would be nice as
well, if that's possible (it wasn't done in SVG 1.1).

Also, the titles of the sections should probably be singular, as they
were in SVG 1.1 (Element Table, Attribute and Property Tables).

Also, the links in the "Contents" section of Appendix K are broken
(href="#").
