A request from the editor

Hi Folks

I would like to make a general point: when you are commenting on a document,
it is much easier for me if you propose some replacement text which resolves
the issue or some additional text as necessary. So if it is possible to do
that, please submit your comments in that form?

I have managed to reduce the open issues

from 76 to 22, so we are getting there, but I think this would help.
