some open 18n issues

In our horizontal review we had some i18n issues open - I really want to
advance core here so I'd like to close them.  It's on me for not
prioritizing them last time, but I'd like to not wait another month to
discuss them so maybe folks can weigh in or even just do what needs doing
and close the issue?

https://github.com/w3c/mathml-core/issues/232 - It sounds like this is easy
to resolve by adding a note... Can we just add it?

https://github.com/w3c/mathml-core/issues/233 - we are intentionally moot
on this subject for v1 because it is a lot and we need to get something
initially stable across the finish line - should we say that and close
this?

https://github.com/w3c/mathml-core/issues/234 - why are they in the spec?
Should we remove them?
