This seems is inconsistant with the SGML declaration used.
After running sgmlnorm on my document, an equivalent document is output.
sgmlnorm folds all my ID's to uppercase.  So now if I have an internal
link specified with lowercase, the link is broken if you follow the String
matching algorithm in the specifications.

It doesn't make sense for to equivalent documents to behave differently.
