Just to point out a couple of things that I *think* are typos :
2.1.4.1, Example 3 : The DTD definition includes Heading.Class, while the
XML Schema definition does not. Heading.class should be added to the XML
Schema definition.
3.3.1 : Last item on the list ("Any combination...") should not be part of
the list at all, but after it.
Sorry if that's been pointed out before/I'm on the wrong list.
