Dear HTML WG (et al.), This is a small errata for the Modularization of XHTML. http://www.w3.org/TR/xhtml-modularization/xhtml-modularization-20001020.html E.5. "Module DTD/examples/myml-default.html not found!" Looks like you wrongly referenced a Server Side Include file which is more than likely meant to be "DTD/examples/myml-defaults.html"!
