Errors in XHTML spec

These errors refer to the 24 August 1999 XHTML 1.0 proposed
recommendation.

Section 4.7 refers to XML's stripping of whitespace in attribute
values.  This only applies to non-CDATA attributes.

In the DTDs and entities referred to in Appendix A

- xml:space attributes are declared as CDATA.  XML requires them to
  be (default|preserve)

- the declarations for amp and lt are not as required by XML.
