Description of insert/@at

The editor's draft of XForms 1.1 has been updated at
http://www.w3.org/MarkUp/Forms/specs/XForms1.1/index-diff.html

The overview of the "at" attribute of insert incorrectly stated that it
was evaluated using the in-scope evaluation context, rather than the first
node and nodeset size of the nodeset binding.

The detailed processing said the latter, as did both descriptions of the
"at" attribute on delete.

This oversight has been corrected. The default of the "at" attribute is
the size of the nodeset binding nodeset, and the correction ensures it is
clear that the default can be expressed with at="last()", even though
expressing the default likely results in a less efficient form.
