8.1.2 - incorrect tool tip for graphic

The graphic showing entry for "Street" has an incorrect tool tip which refers 
to the "title" of the form control. It is the label, not title.

Also the tool tip refers to "street" when the label value is "Street".

Also there is a typo in the second code example: "date ship date".
