Everywhere an attribute is marked Optional, please say what the
default value is.

Please say everywhere what the type of attributes are. For instance
instance@resource. Please list the possible values when they are of an
enumeration.

Please say everywhere if an XPath expression is required to evaluate
to a particula type (eg insert@at must be a number or integer).
