[LC1.1] Changes from XForms 1.0 and examples

These are general comments about things I would have liked while 
reviewing:

1) I struggled to find the differences between 1.0 and 1.1, I guess I was 
expecting a brief intro section like: 
http://www.w3.org/TR/2001/REC-xhtml11-20010531/changes.html#a_changes
I did find the diff marked version, which was a big help.

2) The Submission module is sufficiently getting more complex but found 
very few examples, I'd recommend adding  a few.
