Create table of contents from Steven Pemberton on 2009-10-13 (www-amaya@w3.org from October to December 2009)

I was trying to create a table of contents. I placed the cursor in a new  
paragraph near the top of the document, and selected the option "Tools>Add  
table of contents", and got a dialog box "! Not allowed".

I think an explanation of why not would at least be helpful here. I can't  
see any reason why it shouldn't be allowed.
