Thank you for this. Before the deadline extension Carlos and I had already gone through these comments and here are some notes -- preceded with [LSP]:
We consider it to be ready for publishing. As we have discussed, it is still possible for us to make some adjustments, but I believe we have no major blockers.
Let me know if you think there are still any issues for us to address or if I have forgotten something!

    Brian:
    I am fine with the content of the form, but do have some concerns about the underlying code and how it may affect accessibility of the form. Two examples that stand out:
    - incorrect use of grouping elements (<fieldset>). There are many cases where a single label and input are wrapped in a fieldset, which is not the correct use. There are other cases where grouped content does not properly use the fieldset.
    - using HTML5 validation instead of custom validation. While it works and is generally accessible, if there are multiple fields not filled out, only the first one is indicated and could cause a user to have to submit, fix one field, submit, fix another field, submit, etc. With custom validation all errors could be surfaced at the same time allowing users to fix all of them at once.

[LSP: We had already asked Daniel to do this check for the two pages and we already fixed a few things - this was not an issue, though. I will contact Brian to see if he has any further comments on this or other possible issues. In the meantime, we will fix the first item (about fieldsets), but we believe that it is not a constraint for publishing. This may be updated in the next release, together with the course list page. The validation of the form is on our list to be further discussed, but I believe it will take a little longer.]

    Sylvie:
    In the help for the description field, one can read: "Provide a brief description of this resource (max. 350 chars)."
    I wonder if the word chars is clear enough for anybody wanting to submit this form or if characters would be clearer.

[LSP: Agreed, not sure this wording is more clear - your last suggestion is almost the same as it is now, so maybe it is not that bad. We will see if we can come up with something better, however, the list is not in the scope of this survey - we will still have another one focused on this, so we also see it as low priority for now - but in time to be included in the review.]
