Please find my replies below! 

> Bug: If I go from the Evaluation, to step 1, and then use the back link, it skips the evaluation and takes me back to the start not the evaluation.

Fixed, thanks for reporting.

> Maybe add an expand all/collapse all option? In the "Your Evaluation" page it might be good to add a date or version field to allow people to indicate current versus old versions of the testing.

I like this idea, will try and prototype what this might look like.

Will look into role=progressbar support too, have you seen this in the wild much?
