Sample page failing automated test

Firstly I commend you for the initiative.

I'm a website developer looking at your samples for better insight on WCAG.

I'm looking at https://www.w3.org/WAI/demos/bad/after/survey.html

My problem is the page is failing automated tests.

Note that I'm not endorsing either of the tools. I just use them as a starting point for testing.
