Seems like your are testing the screen reader's support of the form, not the form itself. Did you report your inconsistencies to the developers of the screen reader? If your goal is to test 4.1.2 http://www.w3.org/WAI/WCAG20/quickref/#qr-ensure-compat-rsv on PDF forms, then you need to understand how a PDF programmatically sets the name, role, state, etc. Maybe inspect the tags? That the particular screen reader is or isn't picking up the information correctly or consistently is a screen reader bug, not a PDF Forms compliance issue.
