Approval to Publish Survey - Due Tuesday, May 31st

The List of Accessibility Courses Submission Form butterfly survey (approval to publish)<https://www.w3.org/2002/09/wbs/35532/courses-butterfly/>[1] is open and ready for your review.  We are approving the submission form first so that we can begin populating the list of courses.   Since there were not many changes made after the monkey review survey, this approval to publish survey is only open for a short time and will close on Tuesday, May 31st.
