f2f meeting registration

Hi Brent & Sharron,

I saw AG asking people to register for their CSUN F2F with this survey https://www.w3.org/2002/09/wbs/35422/2019-03_FtF/

I forgot that's how we used to do it. :-)

For EOWG f2F, I think just having people list in a wiki like we do is fine.

I added this to cover the accessibility and food accommodations:
