I think you misread it. What the standard says is "The content of the DigestValue element shall be the base64 encoding of this bit string viewed as a 20-octet octet stream", which is correct. It might help if we added the encoded length (28 characters, of which the last is an equal sign) to the description to prevent further misunderstanding.
