Perhaps there ought to be be a recommendation for what servers should do when
creating a new resource without any Content-Type provided by the client.  LOCK
is the most common case where that can happen, however, and this recommendation
*was* settled on when we came to consensus on how to handle LOCK to unmapped URLs.
