"Add affiliation" does not work

Hi,

I just joined https://www.w3.org/community/sparql-12/. In order to proceed, I had to falsely deny an affiliation to any organization, because I was not able to add an affiliation to my profile. (After a very short display  of a screen which apparently allows to input one's affiliation, I was redirected to another screen "Change my affiliation", with nothing to select whatsoever.

Please let me know, when the issue is fixed, or even better hint me to a ticket system where such issues can be reported.
