we've a couple of issues with our xs:include examples:

* Mono rejects xs:include unless it is at the top of the Schema,
  our XSLT produces examples.wsdl with xs:include half way down

* we're reusing ItemList which means we get a namespace clash,
  guess we need to have a small schema outside of examples.xml
