related to ISSUE-48, expand the collection pattern to include 
attributes included by reference as well as locally defined 
attributes as at present, e.g.:

  <xs:complexType name="MyComplexType">    
    <xs:sequence>
      ... 
    </xs:sequence> 
    <xs:attribute ref="tns:myGlobalAttribute"/>  
  </xs:complexType>
