Standard Steps Edits - New Draft

Here is my list of changes that I made:

* added the p:error step
* renamed 'p:subsequence' to 'p:matching-documents'
* renamed 'p:ns-rename' to 'p:namespace-rename'
* added the p:string-replace step
* renamed p:parse to p:unescape-markup
* renamed p:serialize to p:escape-markup
* added proposed p:count
* added proposed p:tee
* added 'suffix' option to p:label-elements
* clarified the target of p:insert
* removed the use of the 'target' attribute in favor of
  'match' for all
* some language cleanup for references to port names
