Per the minutes of the 2 November 2009 f2f meeting, the mention of
p:namespaces is an error and the rules for statically known namespaces
are changed as follows:

  Unchanged from the implementation defaults. No namespace declarations
  in the XProc pipeline are automatically exposed in the static context.
