See the thread starting at http://lists.w3.org/Archives/Public/spec-prod/2004OctDec/thread.html#9 You are using XMLSpec which is good somehow, but it's also sometimes very unfortunate for the MarkUp, it produces. :) I intend to look at it further but if you can hack the XSLT that would be good.
