Another stab at issue tracking for xml:id

Experience suggests that managing a separate issues document is
tedious and error prone. Building on some work that DanC has done
for tracking LC comments in other groups, I've constructed:

  http://www.w3.org/XML/2004/xml-id/wd-status/status-report.html

Which uses the mailing list to track issues. I've closed a few
threads, but the archives haven't updated so I can't rebuild the page
yet. Hopefully sometime in the next hour or two I'll get to rebuild
the page showing a few closed threads.
