XHTML M12N updated

As per my action item today, I have updated the Disposition of Comments 
document and the Editor's Draft of XHTML M12N.  I did not figure out how 
to mask out the diff marks for the CURIE data types in the schema 
implementation, but they are definitely identical to the version in the 
current, published M12N 1.1 Recommendation.
