I am not sure I like the `<insert>` succeeding with "cat" much either. And
there are already ways of specifying the first and last items:

    <insert context="list" ref="item" at="1" origin="node"/> - inserts at
start
    <insert context="list" ref="item" at="last*()" origin="node"/> -
inserts at end
