WSDL 1.1 Example

Hi all,

Here is the WSDL 2.0 example in Appendix A converted to WSDL 1.2 for
possible inclusion in the SAWSDL document as another appendix
(Appendix D ??)

Maybe in the SAWSDL document, we should talk about WSDL 1.2
compatibility instead of WSDL 1.1, since WSDL 1.1 does not allow
attribute level extensiblity of "operation".
