ws-transfer/fragment test coverage

In looking at the normative statements in WS-Transfer and WS-Fragment, I 
believe that our tests cover them all.  The only exception is the testing 
of the various faults defined - we do not have tests that force each to be 
generated, but I believe that's ok.
