Errata: WS-A SOAP Binding: Two occurances of <wsa:ProblemHeader>

Editorial:

 

The <wsa:ProblemHeader> fault detail element was removed during the CR
phase, yet there are still two incidental mentions [1, 2] of this
element in the spec.
