figure > [caption]:first-child
or
figure > [caption]:last-child

If you're putting your captions somewhere in the middle of your
content, you're on your own.

(Yes, these won't work in all IEs, but then we're running into
ordinary CSS issues that can't be fixed at the HTML level.)
