Resolves Issue #142: Spec's issues index is incomplete, out of date

- Add references to existing issues with the v1-spec label.
- Add new issues with the v1-spec label that were inline TODOs and actually change the spec meaninfgully.
- Remaining editorial TODOs are converted into unlinked issues.  Bikeshed/W3C specs don't have a  "TODO" inline type, and it wasn't worth fighting with Bikeshed to add a custom one.

Merging now, but PTAL @baylesj and can address feedback in a followup PR.
