This issue tracks refinements to the Presentation API messages based on these TODOs:

> presentation-start-request: Add optional HTTP response code to the response?
> presentation-termination-request: Split up reason into reason and whether it was triggered by the user or not?
> presentation-connection-close-event: Why does the Presentation API spec not mention the use of the close message?
> Specify message ordering groups.
