[w3c/browser-payment-api] chore: fix xrefs and dfns (#296)

** Merge this last - or, let me rebase against the other PRs before merge ** 

This fixes most of the cross reference issues in the spec. 

Currently, there are a bunch of `<code>` things that don't link anywhere that need to be fixed. But we can those as we go...
