  Review of PR #944 (#945)

* Replace old <dfn>s with links

* Eliminate <dfn> for "No attestation statement"

Since its text never appears again in the document, change the one link
to it to point to <dfn>None</dfn> instead.

* Add link to "none" attestation statement format from None attestation type
