ReSpec source files plus how to convert to Bikeshed

in order to more easily merge all the FIDO member submission
specs into a single spec, we should use the ReSpec source files, rather
than the published ReSpec output files (which is what the Overview.html
files in the repo are).
this pull request supplies all three originally submitted ReSpec source
files -- one in each of the subdirectories as appropriate.
this PR also contains an initial conversion of
SUBM-fido-web-api-20151120.html to Bikeshed source (index.src.html) and
processed Bikeshed output (index.html). Please see the README.md file for
details. diff'g SUBM-fido-web-api-20151120.html and index.src.html will
illustrate how to do an initial bikeshed conversion.
Further refinement of the bikeshed source is possible -- eg to take
advantage of
further bikeshed features -- but is not necessary for an initial
conversion (in fact one doesn't need to go as far as I did).
