One late note, because this came up in a TAG discussion and it seemed like it might clarify the decision about 2- vs. 3-party model... Back to that notion of the dual purpose of an email address, it being both a means of identifying someone and as a means of communication. If your use for the email address includes communication, the mail service sees who messages are to and from (even with mail encryption). A three-party model therefore only provides privacy benefits if you are using the email address solely as an identifier.
