[w3ctag/design-reviews] Encourage feature owners to list all relevant standards groups. (PR #1044)

And move most questions out of the "Further details" section.


This is meant to avoid the questions of whether particular WGs were consulted that came up in https://github.com/w3ctag/design-reviews/issues/1015#issuecomment-2577114166.
