[whatwg/url] Be clearer that non-UTF-8 code paths are only relevant for a small subset of implementations (Issue #746)

I don't really see a need to revamp the top-level entry points (though basic URL parser -> core URL parser would be somewhat nice), but we should be clearer in a normative sense that not all implementations have to implement all code paths.
