Liking a lot of what others are saying.  To throw in my 2 cents succinctly:

**What was the intention with `userVisibleOnly`?  I don't understand the goal yet.**  

Right now, it seems like it might not be needed.  NotificationAPI already handles the permissions, and I'm missing any meaningful security differences between PushManager and WebSockets.
