https://github.com/whatwg/dom/pull/1027 is where we decided on the design. And that mirrors https://github.com/whatwg/streams/issues/1165#issuecomment-920114795. At least @jasnell was involved on behalf of Node.js.

I don't know where `cause` originates but that was never part of the design and I don't see us adding it now.
