[w3c/manifest] processing a manifest steps refer to "document URL", but doesn't define what that URL is (#946)

In step 4 of https://w3c.github.io/manifest/#dfn-processing-a-manifest a "document URL" variable is used, but that variable is never defined. Is there a missing step that somehow derives that URL from the (otherwise unused) "el" link element argument?
