Re: [w3c/manifest] Make the default scope the origin rather than the parent directory of the start_url (#993)

Tbh, I think we should just do the restrictive behavior (which might just to use the document URL as the scope... or just keep what we have now, which covers the "80% use case"). 

It seems that by just explicitly adding `"scope" : "/"` folks would get the desired behavior.
