Yes, `userVisibleOnly` needs a better spec.

For example I have just received this message from a customer:

> [About Pushpad] Does your API allows to not show the notification if the website has focus?

My first reply was that **silent push is not allowed**... but then I found this and I am really confused:

Please clarify if that is really allowed and what exact actions are considered visible to the user.
