@lknik Thanks for the comment! I don't think this is ever likely to be a danger, as UAs have no way of knowing whether a user may or may not be able to see the image based on whether they are using a piece of software which accesses accessibility APIs. And even if we could tell that (which we can't), we wouldn't know whether they were likely sharing their screen with someone else at any given moment. The use of accessibility APIs really seems like too weak a signal to use for such a drastic measure, compared to something like a user preference not to download images.
