I want to avoid fullscreen elements covering anything else in the top layer, e.g. dialog.

I don't think it is necessary to be a common rule in the top layer system. I believe it makes sense to have dialog on top of fullscreen element, but not versa vice.

BTW, I don't think I mentioned anything about the hierarchy requirement in my last comment. I just said the topmost element in the top layer must have fullscreen flag set.
