[w3c/webcomponents] Move HTML Module Record definition from ES spec to HTML5 spec (#796)

Move HTML Module Record definition and associated algorithms to HTML5 spec per discussion [here](https://github.com/w3c/webcomponents/issues/783).  Add hyperlinks for calls that are now cross-spec.  Also some minor wording/typo fixes.
