[w3c/manifest] [Explainer] Add simple example with name and icon (#527)

The proposed example looks like it is very common on simple websites with manifests incuding:
* http://www.boatersnet.net/manifest.json
* http://pulseconsultant.com/manifest.json
* https://www.fallkniven.com/wp-content/plugins/fallkniven-advanced-customizations/favicons/manifest.json
